At minimum, cover new subscription charging and failed-payment handling. Add upgrade/downgrade and proration cases once you actually offer multiple tiers.
Automate the ones you can run repeatedly against a sandbox billing environment. Manual testing is reasonable for less frequent journeys like annual-to-monthly switches early on.
