We use six commonly required categories: reported symptoms, medical history, current medications, allergies, reason for visit, and insurance/admin information. Your intake process may reasonably use different categories — treat these as a practical starting checklist.
Those two fields carry direct safety implications if a clinician acts without them, so the completeness score applies a 20-point penalty per missing field rather than treating every category as equally weighted.
It's a weighted sum of self-reported severity, how long symptoms have persisted, and the number of risk flags raised, scaled to 100. It is meant to help staff sort intake volume, not to make a clinical triage decision.
