Is Care Check Forms medical or clinical advice?

No. This is a form-design planning aid, not medical or clinical guidance, and it does not build, host, or store any actual questionnaire responses — the score is calculated entirely from what you enter, in your browser.

Why does open-ended question share increase risk?

Free-text answers are harder to triage automatically and more prone to inconsistent detail from patients, so a high share of open-ended questions raises the completion/quality risk score relative to structured or scaled questions.

What counts as a 'validated scale'?

A validated scale is a published, tested instrument such as PHQ-9 for depression screening or a standard pain scale, as opposed to custom questions your team wrote from scratch. Use whichever is clinically appropriate for your use case.

Does this build the actual questionnaire?

No. It only scores the design parameters you describe — you still need to build the actual form in your intake or survey tool.

How long should a follow-up questionnaire really be?

As a rough guide, keep most check-in questionnaires under 10 minutes; anything longer meaningfully increases drop-off and rushed answers, which is why length is one of the heaviest-weighted factors here.
