Can I use this for more than one medication or symptom?

Yes, run a separate entry for each distinct experience or medication, since the severity and timing patterns are specific to each one.

Does this replace an official adverse-event report?

No. If required by your country's regulations or your healthcare provider's process, you should still file a formal adverse-event report through the appropriate channel — this tool only helps you organise your own notes.
