How is the escalation score calculated?

It combines a weighted category score, a harm-level score, a fixed recurrence add-on, and a capped time-elapsed factor, then normalizes the total to 0-100. Category and harm level are the heaviest drivers.

Does this replace our incident-management system?

No. It is a quick, standalone priority estimate for a single incident. Your incident-management or QMS platform remains the system of record for reporting, assignment, and closure workflows.

Why does time elapsed matter less than category or harm?

An incident sitting unresolved for a long time is a real signal, but it should not outweigh what actually happened. The time factor is capped so a low-harm incident cannot outscore a severe one just because it aged.

What counts as a "recurrence"?

Mark it as a recurrence if the same incident type (not necessarily the same device or location) has happened before. Recurring incidents typically indicate an unresolved root cause and are weighted accordingly.

Is this tool a medical, legal, or compliance judgment?

No. This is an operational and administrative planning aid to help prioritize follow-up, not medical, clinical, or legal advice. It does not diagnose anything and does not store or transmit any patient data — all calculation happens client-side in your browser with no backend involved.
