Unstated scope is the most common source of feature creep — writing down what's excluded makes the boundary explicit for both humans and AI coding assistants.
Something that, if false, would change the design — like an assumed file size limit, user role, or data format. Vague assumptions like 'users will like this' don't help.
Each of the four fields (name, requirement, assumption, exclusion) contributes up to 25% based on whether it has enough detail (roughly 15+ characters), with partial credit for shorter answers.
