Does this tool decide which medications to keep or stop?

No. This tool only calculates a risk score to help prioritize review. Every discrepancy it flags still requires a pharmacist or clinician to confirm the correct action — it never makes or suggests an autonomous clinical decision.

Why do undocumented discontinuations weigh more than dose changes?

An undocumented discontinuation carries a higher risk of an unintended therapy gap, since there's no recorded rationale to confirm it was intentional. Dose changes are often deliberate and lower risk, so they're weighted less heavily.

Why does discharge score higher risk than admission for the same numbers?

At discharge there is typically no immediate next clinical checkpoint to catch an error, so the same discrepancy carries more downstream risk than it would at admission, where subsequent visits offer more chances to catch it.

Is this a substitute for a formal medication reconciliation process?

No. It's a quick, informational prioritization aid based on the counts you enter. It does not replace a full pharmacist- or clinician-led reconciliation against verified medication records.
